Great Plains Health Data Network

The Community HealthCare Association of the Dakotas and the Wyoming Primary Care Association are forming the Great Plains Health Data Network after receiving a three-year grant from the Health Resources and Services Administration. The $1.56 million grant will help bring together 11 community health centers and 70 sites that serve more than 111,000 patients in underserved and low-income areas across South Dakota, North Dakota and Wyoming. The network will invest in shared technology and technical staff to help improve population health management, data analytics, data interoperability and patient access to their health information.
